As a Kiwanis Member, you're now entitled up to $3,000.00 in NO-COST Accident coverage. This base benefit is an important entitlement extended to valued Members - compliments of The Kiwanis Member Benefits Program.

Benefits Are Paid in Cash

Accidental death benefits are paid for the coverage amount ($3,000.00 for the NO COST to you coverage plus the amount of
expanded coverage you choose) if you or a covered spouse suffer bodily injuries as a direct result and from no other cause than that from the accident, which results in loss of life. The accident must happen while the person is insured and the loss must occur within 365 days after the date of the accident.

TERMS

The Kiwanis Accident Plan cannot pay a benefit for a loss which is caused by, results from, or contributed by: Suicide, attempted suicide or intentionally self-inflicted injury, while sane or insane (in Missouri and Colorado while sane) declared or undeclared war or any act of war; full-time military service; participating in a riot; committing an assault or felony; sickness or its medical or surgical treatment, including diagnosis; bacterial infection except through wound accidentally sustained; operating or riding in any kind of aircraft except as a fare-paying passenger on a regularly scheduled commercial flight or as a passenger in a transport plane operated by the Air Mobility Command (AMC) of the United States of America; alcohol intoxication, as defined in the state where the accident occurred; taking of any drug, medication, narcotic or hallucinogen, unless as prescribed by a physician; taking of alcohol in combination with any drug, medication or sedative; voluntary gas inhalation or poison voluntarily taken, administered or inhaled; riding or driving as a professional in any kind of race for prize money or profit.

Terms and conditions of coverage are set forth in Policy numbers, non-contributory policy MZ0927408H0000A/0002A; contributory policy MZ0927408H0001A/0003A. No one can be covered more than once for no-cost and paid coverage under this policy. Additional information is contained in the Certificate of Insurance which is issued to all persons who have purchased any expanded coverage. Premiums for the no-cost portion of your insurance coverage are paid by Kiwanis Endorsed Member Benefits as a benefit of your membership. The no-cost Member coverage is effective for 12 months and does not automatically renew. This plan may not be available in all states. Availability of this offer may change. Kiwanis member dues or contributions are not used for this promotion, program, or any other related expenses.

HOW BENEFITS ARE PAID Benefits for loss of life are payable to your beneficiary. If no beneficiary has been named, benefits will be paid to: your spouse; your children; your parents; your siblings; or your estate (in that order). Benefits for loss of your spouse's life are payable to you.

Exclusions: The Policy does not cover any loss that is caused by, results from, or contributed to by: suicide, attempted suicide, or
